Jamie worked as a forklift driver at Hive Logistics. On 1 November 2018, Jamie injured his back when he lifted a 25-kilogram box.

He has been certified unfit for work since his date of injury.

Jamie lodged a WorkCover claim immediately after his injury which was accepted by the WorkCover insurer.

On 5 June 2020, the WorkCover insurer provided a notice to Jamie proposing to terminate his entitlements to further weekly payments of compensation effective 6 August 2020.

The grounds for termination were that the claimed incapacity for work was no longer materially contributed to by a work-related injury.

Jamie attends your office on 5 September 2020 and provides you a copy of the insurer's decision.
